Quality failures on commercial projects regularly trace back to the same three issues:

Construction quality management is the system the head contractor puts in place to prevent those failures, ensuring all works are delivered to the standards required by the contract, the design, and applicable Australian Standards.
In this article, we explain how quality management works on a commercial project: what a QMP must contain, how ITPs (inspection and test plan), hold points, and NCRs (non-conformance reports) function in practice, and what quality obligations mean for payment claims and practical completion.

What is construction quality management?
Construction quality management is the structured system through which a head contractor plans, documents, monitors, and verifies that all works meet the contractual and technical standards required.
Without that system, quality obligations become reactive rather than planned and inspections get missed.
On a commercial project, quality management requires coordinating multiple trade packages, subcontractors, and inspection regimes simultaneously, while maintaining the documentation trail the superintendent will rely on to assess progress claims and practical completion.
Construction quality management and contractual compliance
Construction quality management directly influences how contractors stay compliant with the terms of the construction contract.
Under AS 4000 and AS 4902, the head contractor carries explicit quality responsibilities tied to payment and practical completion. Meeting those obligations requires a structured system in place from the date of contract.
Under most Australian standard form contracts, that means submitting a QMP within a set number of days of executing the contract, maintaining quality records throughout the works, and making those records available to the superintendent on request.
Late or incomplete compliance with any of those obligations can affect the superintendent's assessment of both progress claims and practical completion.
How QA and QC work together on a commercial project
Quality assurance and quality control are distinct functions that operate together under the quality management umbrella.
Quality assurance:
Quality assurance covers the proactive processes put in place before and during construction to give confidence that quality requirements will be met.
Quality control:
Quality control covers the verification activities that confirm work has been completed to the required standard.
QA without QC is just paperwork, and QC without QA produces records that are hard to stand behind when the superintendent starts asking questions at practical completion. The two functions need to run in parallel from contract award for the head contractor to meet their quality obligations.
How the quality management plan governs project quality
The head contractor's Quality Management System (QMS) sets the organisational framework for quality across all projects. The Quality Management Plan (QMP) applies that framework to a specific contract.
It is the QMP that the superintendent assesses, that ITPs and NCRs sit under, and that governs how quality is managed on the project. Project teams use it to coordinate inspections, track quality records, manage subcontractor compliance, and support practical completion.
A compliant QMP addresses the following:
Project quality objectives
Project quality objectives are the measurable quality outcomes the head contractor commits to achieving on this specific contract, including tolerances, finish standards, and any client-specified performance criteria that will be used to assess practical completion.
Applicable standards and codes
This includes the Australian Standards, NCC requirements, and any client-specific standards that govern the works, with enough specificity that site teams and subcontractors know which version of each standard applies and where it governs their scope.
Organisational structure
The organisational structure names individuals with quality responsibilities assigned, including who prepares ITPs, who conducts inspections, and who holds sign-off authority. Unnamed roles are not sufficient; the superintendent needs to know who is accountable.
Activities requiring ITPs
Lists every trade package or work activity requiring a formal inspection and test plan, drawn from the contract requirements and the head contractor's own risk assessment. The list should be established at the start of the project, not built incrementally as packages are awarded.
Hold and witness point schedule
The hold and witness point schedule is a consolidated register of every hold and witness point across all ITPs, with notification requirements, responsible parties, and lead times identified. Managing this register actively is what prevents programme delays caused by missed or late inspection notifications.
NCR procedure
This outlines the process for identifying, recording, actioning, and closing non-conformances, including timeframes, escalation paths, and the standard of evidence required to close each NCR. A procedure that does not specify closure requirements is difficult to enforce against subcontractors.
Document control procedure
Document control procedures outline how quality records are stored, version controlled, and made available for superintendent audit and payment claim substantiation. Records that cannot be located or verified when needed carry the same risk as records that were never created.
Subcontractor quality obligations
This section of the QMP defines the specific quality requirements subcontractors must meet, including their own ITP submission requirements, hold and witness point obligations, and how their compliance will be monitored and recorded by the head contractor throughout the works.
How quality is managed on-site
The QMP establishes the quality system, but its the ITPs, hold points, witness points, and NCRs are the instruments through which that system operates across active works.
Each serves a distinct function in the delivery and verification of compliant work. Here’s how the three instruments play out in enforcing the QMP.
Inspection and test plans
An ITP is a document that sets out the sequence of inspections and tests required for a specific trade activity. It identifies:
- What is being checked
- Against what standard
- By whom
- At what point in the sequence
ITPs must be prepared and submitted before the relevant work commences. A retrospective ITP cannot confirm that inspections occurred at the right points in the sequence or that hold points were observed before work proceeded. Where the superintendent has reason to doubt the integrity of the inspection record, that doubt falls on the head contractor.
Subcontractors must prepare ITPs that comply with the head contractor's QMP requirements, and those ITPs must be reviewed and accepted before work begins.
On a large project with 20 or more active trade packages, managing ITP submission and sign-off across the programme is a significant administrative task. Without a dedicated tracking system, packages slip through, work starts without accepted ITPs, and the head contractor's quality records become difficult to defend.
Hold points and witness points
A hold point is a mandatory stop in the work sequence where the superintendent must inspect and provide written approval before work can proceed. A witness point requires the superintendent to be notified and gives them the right to attend, but work can proceed if they do not attend within the notified timeframe.
Failure to comply with either has real-world consequences, though not of the same kind or severity.
Proceeding through a hold point without superintendent sign-off is a contract breach and can require completed work to be uncovered at the head contractor's cost. Witness points don’t carry an equivalent contractual consequence, but failing to notify the superintendent removes their right to attend and can create disputes about whether the inspection was properly conducted.
Managing hold and witness points becomes a sequencing and coordination task as much as a compliance one, particularly where multiple trades, inspections, and programme dependencies overlap.
A missed notification can stall a trade package while the superintendent is chased for attendance, and a backlog of uninspected hold points can create programme pressure that leads to the kind of shortcuts that generate NCRs.
Head contractors must identify which applies to each activity in the ITP, build notification lead times into the construction programme, and track outstanding approvals actively.
Non-conformance reports
An NCR is a formal record raised when work, materials, or a process does not meet the specified requirements. The typical causes of non-conformance include:
- Workmanship that does not meet the specified standard or tolerance
- Materials installed that do not match the specification or approved submittal
- Work proceeding past a hold point without superintendent sign-off
- Subcontractor methods that deviate from the accepted ITP sequence
When non-conformance is identified, the head contractor must raise an NCR, typically using a workflow such as:
- Identify and record the non-conformance
- Determine the cause
- Specify corrective action
- Assign responsibility and a due date
- Verify completion
- Close with a record of the outcome
Each step of this process needs to be documented.
An NCR that is raised but never closed tells the superintendent that a non-conformance was identified and then ignored, giving them grounds to dispute what constitutes the defects list and what was identified and rectified during construction.
The head contractor also loses the ability to back-charge subcontractors for rectification work if NCRs accumulate without formal corrective action records.
Back-charging a subcontractor requires demonstrating that the defect was identified, formally notified, and not rectified within the required timeframe. Without a closed NCR trail, that case is difficult to make and rarely holds up in a dispute.

How project teams manage quality day to day
On a live project, quality management is more about on-site coordination than a documentation exercise. The QMP and ITPs set the framework, but someone on the project team has to drive compliance across every active trade package, every week.
For a contracts administrator or site engineer, that means maintaining a live ITP register that tracks submission status, review outcomes, and hold point approvals across all packages. Issues appear fast on a busy site, and a missed submission or outstanding sign-off that isn't caught early can stall a trade or create a compliance problem that is only spotted at practical completion.
The day-to-day quality tasks a CA or site engineer is typically managing include:
- Tracking ITP submission and acceptance status across all active trade packages
- Monitoring outstanding hold point approvals against the construction programme
- Reviewing subcontractor inspection records and confirming sign-offs are completed before work proceeds
- Raising NCRs when non-conformances are identified and tracking corrective action through to close-out
- Maintaining the quality register so records are current and available for superintendent audit
Ensuring subcontractor compliance is one of the most persistent challenges. Enforcing ITP submission deadlines, holding subcontractors to hold point obligations, and ensuring their inspection records meet the QMP standard requires consistent follow-through from the project team throughout delivery, not just at handover.
How quality management affects practical completion
The superintendent's assessment of practical completion relies heavily on the quality record the head contractor has maintained throughout the works. A complete set of closed ITPs, resolved NCRs, and QMP-compliant documentation gives the superintendent what they need to certify, and any gaps in that record give them grounds to withhold or qualify certification.
Practical completion certification
The superintendent is entitled to assess whether the works have been completed in accordance with the contract before issuing a certificate of practical completion, but that assessment isn’t limited to a physical inspection.
The superintendent can and will review the quality documentation trail (ITP sign-offs, hold point approvals, NCR closures) as part of that process.
A head contractor who has managed quality actively throughout delivery is in a strong position
Payment claims
Under most Australian standard form contracts, the superintendent assesses progress claims against the works as executed.
Where quality records don't substantiate that work was completed to the required standard, the superintendent has grounds to reduce or defer certification of that portion of the claim.
Defects liability period
A clean handover reduces exposure during the defects liability period.
If your quality record clearly shows what was inspected, what non-conformances were identified, and how each was resolved, then the scope of the defects liability period will be defined.
If it doesn't, disputes about what constitutes a defect under the contract and what was already identified and rectified during construction are harder to resolve and more likely to escalate.

A head contractor who sets up the QMP properly, manages ITPs and hold points actively, and closes NCRs as they arise is not just meeting a contract obligation. They are building the documentary record that protects their position on payment claims, supports a clean handover at practical completion, and makes non-conformance disputes with subcontractors far easier to resolve.
